Mission Support Alliance experienced an unplanned outage of the Supervisory Control and Data Acquisition (SCADA) system, which monitors the electrical power distribution substations located on the Hanford Site. The outage happened during a routine preventive maintenance activity. The SCADA system is equipped with Uninterruptable Power Sources (UPS) that are designed to prevent outages by supplying battery power for a limited time prior to the initiation of substation backup power, however the batteries in the UPS failed and therefore did not operate as designed, resulting in the outage. When performing maintenance on UPS equipment, it is recommended that replacement parts be in accordance with the vendor's specifications and the preventative maintenance activity be completed prior to the device's known failure point instead of operating in a run-to-failure approach to prevent future outages.
