The attached Operating Experience Level 3 document provides information about recent gasket failures in flanged joints of High-Density Polyethylene (HDPE) piping. The National Nuclear Security Administration Production Office at Pantex provided information on HDPE piping joints and two recent failures. Recommendations include re-torqueing flange bolts 24 hours after gasket installation and coordinating with the gasket manufacturer when selecting material.
