Several employees reported a fire in a construction roll-off dumpster. The fire alarm in the building was pulled by a building employee, however the fire department responded to the report of the smoke before the fire alarm enunciated. A cascade of situations and contributing factors are identified including: Improper disposal of smoking material, a non-functioning fire alarm system, poor audibility of a public address system, and indifferent personnel evacuation were identified as contributing factors.
