Work was being done on the installation of water fountains when an electrician along with the Hazardous Energy Specialist, the Craft foreman and a Building Specialist completed a Lockout/Tagout walk down to ensure proper components and zero energy points had been identified for the scope of work. Approximately three weeks later when the work was being executed it was discovered that a simple Lockout/Tagout tag had the incorrect breaker number identified and subsequently placed on the wrong circuit. This resulted in a circuit trip when the electrician started to cut the energized wires with an insulated tool. No one was harmed during the course of work.
