Changing personnel roles and positions if not carefully and completely integrated with associated required training and qualifications can lead to lapsed or incomplete requirements. An annual Lockout/Tagout program assessment discovered multiple personnel were either continuing to perform Lockout/Tagout with a lapsed qualification, or had not fully completed Lockout/Tagout qualification before assuming their Lockout/Tagout functions. This led to a suspension of all Lockout/Tagout activities until qualifications were confirmed, and to the declaration of a recurring ORPS Report.
