A brand-new electric space heater failed when it was turned on - the owner heard a "bug zapper" sound, saw a red glow, and saw and smelled smoke emanating from the interior of the unit. The owner immediately unplugged the unit, exited her office, and made the appropriate notifications. Fortunately, there was no flame and her fast, correct response may have prevented a fire. Disassembly and examination of the failed heater did not identify a definitive cause.
