After performing visual verification on waste materials at an Offsite Waste Generating Facility, two CHPRC Waste Acceptance personnel could not clear the Personnel Contamination Monitors upon exiting the Radiologically Controlled Area. Assumptions in communications and follow up response led to delays in follow-up personal surveys and conflicting information involving the actual status of the issue.

Lesson Learned: Established channels for internal and external communication must be implemented in order to support notification and efficient response to unexpected conditions occurring offsite.
