Advanced Test Reactor Utility Area Operations personnel performed an annual test of the firewater system to validate the fire water system operating parameters in accordance with established Technical Safety Requirements. The purpose of the test is to validate the ability of the deep well to supply cooling water via the Emergency Firewater Injection System during the loss of a cooling water accident. The fire water system is design rated at 160 psi. During the performance of the test, personnel inadvertently missed a procedure step requiring the deep well to be secured and performed other steps out of sequence which resulted in an over pressure condition that exceeded 200 psi.
