At the Materials and Fuels Complex, a Health Physics Technician issued a Powered Air Purifying Respirator to a worker who was planning to enter a High Contamination Area / Airborne Radioactivity Area. The worker put together the hood assembly and failed to perform an adequate inspection of the gasket seal that prevents leakage of contamination into the hood. Prior to entering the area, the worker was stopped by an observer who noticed that the gasket was missing from the connection. The missing gasket was found on the floor in the area where the respirator was assembled.
