During a monthly inspection of the luffing crane at a Hanford Facility, an Aerial Boom Lift malfunctioned causing two individuals to become stranded approximately 90 feet in the air. Since a similar scenario had been evaluated during work planning preparation, the workers were rescued in an orderly manner.

Best Practice: Reviewing potential upset conditions during work planning will prepare the work team to address those upset conditions in a timely manner when they occur. Including outside organizations during work planning and walk-downs for upset conditions will help with timely responses, work location control handoffs, and getting workers to safety quickly.
