Use of a new powered air-purifying respirator (PAPR) without adequate hands-on training resulted in multiple issues and failures. Use of a new PAPR was implemented by waste retrieval workers. Training was provided prior to use but did not require new users to demonstrate proficiency in the inspection, assembly and operation of the units, relying instead on the assumed similarities between other PAPRs and the new one.
