Inadequate assembly and inspection of a scaffold by a Construction Rigging Competent Person resulted in the movement and displacement of plywood when stepped on by an employee during dismantlement activities. One section of the plywood decking cantilevered upwards on the opposite end when an employee stepped upon a seam where 2 sections of the plywood were abutted. The Construction Competent Person responsible for sign-off acknowledged that he knew that the abutted plywood needed support, but he became pre-occupied and failed to install support for the section of unsupported plywood before he signed off the scaffold tag.
