A near miss occurred when an ongoing issue rolling heavy loads on pallet trucks over an uneven freight elevator threshold gap went unreported. Action to resolve the issue didn't occur until a 2,000 lb. load fell off a pallet truck into the elevator. Lessons learned: when an ongoing ESH issue is recognized, contact your ESH POC for assistance; take the time to really understand the problem and related risks; and develop an appropriate long-term solution. In this case, there are two relatively simple solutions.
