Regularly assess the structural stability of ecology block walls. If integrity appears compromised, cease work around the wall until repaired or rebuilt.

The Washington Fatality Assessment and Control Evaluation Program has published a new Fatality Investigation Report. The new report describes an incident where a heavy equipment operator was killed when he was crushed by an "ecology block" weighing nearly 4000 pounds. These reports describe work-related fatal incidents and list some requirements and recommendations that might have prevented the incident from occurring. We hope that they are used for formal or informal educational opportunities to help prevent similar incidents.
