In July 2007, the shoes and/or clothing of several workers were discovered to be radiologically contaminated following a job to "sleeve" a radiologically contaminated component into a valve pit.
Suggestions to prevent events - Use mock-ups to familiarize workers with new work techniques, ensure that contamination control techniques are discussed during pre-job briefings, do not assume that workers are proficient in all work techniques. Use a questioning attitude, discuss planned work techniques with workers during pre-job walk-downs.
