Radiological planning for entry into a shipping container was not conservative, which resulted in contamination of personnel and equipment.

Lesson Learned: A container surveyed as Department of Transportation (DOT) "empty" allows the container to be transported, but does not have sufficient characterization to verify that there is no contamination present. Controls should be institutionalized to ensure that containers are treated as though contamination is present until proven otherwise.
