Work that introduces changes and potential hazards, an out-of-specification condition, or some other unexpected condition such as unfinished work, needs to be documented and communicated.

Six lengths of one-quarter inch copper tubing unexpectedly fell from wall penetrations approximately 15 feet above the floor during a crane inspection. The crane inspectors were approximately 10 feet away from where the tubing landed. A second event involved approximately 800 gallons of low-conductivity water that leaked onto a floor of a lab. Fortunately, neither event injured any personnel.
