Employees were tasked with relocating a shielded sample vault from one lab to an adjacent lab. The sample vault weighed 1700 pounds. The workers used pry-bars and wedges to lift the vault and place it on equipment skates that were sufficiently rated for the load. The vault was manually pushed out of the lab and down the corridor. As the workers were pushing the vault into the new lab location, the wheels of the equipment skate caught on an uneven floor transition causing the vault to topple forward.
