Plan for contingencies (what-if scenarios) even when there is a long history of successful work execution. During AY-102 Tank Annulus video inspections, live video was temporarily interrupted and (and unknown to personnel) the camera made contact with tank waste. When the camera was retrieved from the annulus the contact dose rates were much higher than expected. Previous history with video inspections had been so successful that the existing (repetitive use) work plan documents did not include dosimetry considerations for a camera making contact with waste.
