A miner finished refueling an excavator and was in the process of stepping down onto the track to lay the fuel nozzle down when he fell. The miner could not recall if he either missed the step or his hand slipped off the handrail. He fell while in a bent over position and landed head first on the barge deck. The miner suffered serious injuries even though he fell only 4 feet.
