PremAire harness ensembles and a Scott SCBA harness and regulator were radiologically cleared from a Contamination Area and later found to be contaminated. Some of the items were found on-site and others had been transported to off-site vendor locations.

Lesson Learned: Document and validate key assumptions during development and review of clearance survey plans to radiologically clear (release) items associated with respiratory protection. Select the appropriate confidence level for clearance of material and equipment. Provide appropriate training to the people who prepare, review, and approve clearance survey plans.
