During prior decommissioning efforts, a beam clamp type hanger and a 12-foot long support rod were left attached to a ceiling beam after the lighting system it was supporting had been removed. In October 2015, a collocated activity caused the rod to fall, nearly hitting a worker.

Lessons Learned: Removing pieces and parts of facility systems can create insidious hazards that could go unnoticed and uncontrolled. During scoping visits or upon entry into a work zone it is important to assess site conditions to ascertain the presence of site hazards that may be a danger to personnel entering or working in the area.
