An electrician was performing an isolation cut through a bundle of wires in an electrical panel. The bundle included an energized wire that had not been previously identified. Cutting the energized wire resulted in the release of hazardous energy and burning the cutting tool.

Lesson Learned: Follow the practices of verifying each wire is de-energized before individually cutting each wire. The configuration of electrical panels may change over time and can contain undocumented post installation added electrical components that require additional rigor when determining Lockout/Tagout boundaries.
