Work to re-certify an elevator was performed without identifying all of the controls for hazards discussed in the manufacturers instructions. Although the work was completed without incident, several discrepancies existed between the work package and the instructions provided in the manufacturers manual.

Lessons Learned: When facility support develops a work document for use by subcontractor personnel, planning must address all applicable hazards. To achieve this, both vendor input and manufacturer documentation must be reviewed for hazards and applicable controls.
