A worker at the Alpha Gamma Hot Cell Facility was unable to remove his arm from a set of gloves inside the gloved repair area when a second worker began operating an in-cell robot located in the same area. The first worker was not in the second workers line of sight. The resulting motion caused the robot to pin the toolbox that the first worker was holding, to the glovebox wall preventing the worker from removing his arm from the glovebox without releasing the toolbox. After a few seconds, the robot was operated in the opposite direction allowing the worker to reposition the toolbox and release his arm from the glovebox gloves. Limited communications between the two workers occurred prior to the operation of the robot and there were no established communications (e.g., radio or verbal) while the robot was in operation.
