An employee, operating a forklift, was in the process of loading a pallet of ceiling tiles onto an unoccupied utility pickup truck when a forming spike that was already in the truck bed pierced the box, cab and drivers seat of the truck. Fortunately, a best practice established by Utilities and Facilities Operations does not allow employees in remain in vehicles during loading operations. The practice prevented an injury from occurring in this event.
Lesson: Examine work areas for any latent hazards that may be in the line of fire of fork-lift operations to determine if the potential exists for worker injury in drop and strike zones.
