A roofing subcontractor had reported that he had taken positive actions to secure all loose roofing materials on the roof due to pending high winds and incoming inclement weather. The subcontractor then proceeded to secure material on the ground and left the site. Following the event the subcontractor stated that they had left a sheet of material on the roof to help hold roofing material that had not completely adhered.
Lessons Learned: Workplace environmental conditions (abnormally strong, gusty winds) experienced during this event exceeded the hazard controls implemented. As a result of this event, additional "tie-in" controls over those required will be incorporated into future roofing Subcontract safety requirements.
