Hanford Fire Department personnel missed a step in a maintenance test procedure that contributed to a Technical Safety Requirement violation at a Hanford nuclear facility. The causal analysis for this event identified problems with personnel training, procedure and data sheet development and control, and field supervision. Management assessment had not identified these conditions for several years.

Lessons Learned: Well-trained personnel, accurate procedures and corresponding data sheets, and appropriate field supervision are essential elements of an effective maintenance program. Periodic management assessment of these elements is necessary to ensure that the program is implemented with sustained excellence.
