The attached Operating Experience Level 3 (OE-3) document provides information on a safety concern related to roll-up doors that fail unexpectedly and endanger workers at Department of Energy (DOE) sites. Several DOE events demonstrate the danger of a roll-up door when mechanical failure occurs. Roll-up doors used in industrial applications, such as at DOE facilities, are heavier than residential doors and require preventive maintenance. Accordingly, the OE-3 document provides recommendations for the safe operation of roll-up doors and emphasizes the importance of preventive maintenance.
This OE-3 document is being issued as part of the Departments Corporate Operating Experience Program to facilitate the sharing of lessons learned from operating experience and the prevention of adverse operating incidents.
