The attached Operating Experience Level 3 (OE-3) document provides information about a safety concern related to the dangers of items falling from heights into spaces normally occupied by workers at Department of Energy (DOE) facilities. This document reviews recent events that emphasize the importance for both workers at height and workers below to know the danger and steps that they must take to remain safe. The OE-3 document also provides recommendations to prevent injury from objects falling into a normally occupied space.

This OE-3 document is being issued as part of DOEs Corporate Operating Experience Program to facilitate the sharing of lessons learned from operating experience and the prevention of adverse operating incidents . Please provide the OE-3 document to your managers and staff for their operational awareness and application .
