On July 24, 2013, at the H Tank Farm, Savannah River Site, an Operator was inadvertently injured with a puncture wound to the finger.

While handling normal Step-Off-Pad waste, the wire reinforcement of a plastic suit hose penetrated through the bag and punctured the Operator. The plastic suit hose was previously cut for size-reducing; however, the sharp wire was not protected.
