A field splice on a new installation of an electrical service was not completed, resulting in an energized 480 volt power cable being uncontrolled. Installation of the new electrical service was interrupted prior to completion and put on hold. After a two year delay, walk-downs conducted as part of replanning the work package failed to identify the missing field splice.

Lessons Learned: When work is resumed, after an extended delay, a very thorough and effective evaluation of work status is essential for the safe performance of work. Personnel must understand their roles and responsibilities for an effective work site walk-down to ensure potential hazards are identified and controls developed.
