The accident investigation of a scissor lift that tipped over at Strategic Petroleum Reserve discovered that the scissor lift had been rented and that manufacturer-specified instructions and warnings had not been incorporated into the project work plans or safety and health plans and that the work crew did not understand or disregarded the warnings provided with the manual and posted on the equipment.

Management and work crew members were confident that they knew how to operate a scissor lift safely and operators were certified under an in-house training program. Using a scissor lift for performing abrasive blasting inside a tank was a new application of scissor lift use for them and they failed to recognize that the task created a new hazard, that of lateral forces that could exceed the tolerance limits of the scissor lift. Lessons Learned: Equipment-specific operators manual and label requirements and operating instructions need to be incorporated into work control documents and practices.
