Management and work crew members were confident that they knew how to operate a scissor lift safely and operators were certified under an in-house training program. Using a scissor lift for performing abrasive blasting inside a tank was a new application of scissor lift use for them and they failed to recognize that the task created a new hazard, that of lateral forces that could exceed the tolerance limits of the scissor lift.
