While management planning attempted to address the disruptions associated with a major organizational change, planning for that transition did not consider the impacts of the removal of key personnel, no turnover, and continuing to try to disposition waste without the knowledgeable resources. Thus, management did not sustain adequate controls throughout the duration of those changes.
