A transposition error in a work scheduling spreadsheet caused collection of five extra groundwater samples including one that produced an unexpected result for hexavalent chromium. Additionally, 12 required samples were not collected as a result of the spreadsheet error. The errors were discovered soon enough to be corrected however, the unexpected result required additional regulatory notifications and re-sampling.
