On February 8, 2012, the D-4 100N project was using a Komatsu PC 800 excavator with a hammer attachment, to break up the 181-NE River Water Pump House concrete floor. During hammering activities, a small piece of rebar was sheared and propelled into the window. The glass was shattered and the rebar fell into the cab, however the operator was not struck by glass or rebar and was not injured.
