While excavating a trench to join new grid transformers to existing 11KV cables, an excavator bucket damaged one of the cables. The 11KV circuit breaker that should have opened within 1.4 seconds to isolate the fault took 50 seconds to open. This was a watershed utilities event and corrective actions are being taken to prevent recurrence in the future. These actions include assessment of asset condition, adoption of higher standards, and strengthening of organisational learning.
