An evaluation to address the potential hazards of unsecured objects staged or stored too close to a negative elevation work area is critical to personnel safety. At the Materials and Fuels Complex Hot Fuel Examination Facility, an operator located in a below grade work area was struck by a torque wrench that had fallen off the back of a truck trailer. The wrench, weighing roughly five pounds, fell approximately nine feet contacting the operator below.
