During floor decontamination activities, a HEPA filter vacuum used to collect debris and dust failed. Work was being performed in a containment tent with a portable/temporary radioactive air emissions unit that exhausted the air to the atmosphere through a HEPA filtration system. Although it was determined that there was no release to the atmosphere when the vacuum failed, filter failure may lead to contamination spread.
